How Do I Qualify?
In order to get your own medical marijuana card in the state of Missouri, you will need to be diagnosed with one of the following qualifying conditions:
Cancer
Epilepsy
Glaucoma
Intractable migraines unresponsive to other treatment
A chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with multiple sclerosis, seizures, Parkinson’s disease, and Tourette’s syndrome
Debilitating psychiatric disorders, including, but not limited to, post-traumatic stress order, if diagnosed by a state licensed psychiatrist
Human immunodeficiency virus or acquired immune deficiency syndrome
A chronic medical condition that is normally treated with a prescription medications that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication
A terminal illness
In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to, hepatitis C, amyotrophic lateral sclerosis, inflammatory bowel disease, Crohn’s disease, Huntington’s disease, autism, neuropathies, sickle cell anemia, agitation of Alzheimer’s disease, cachexia, and wasting syndrome.

During your appointment, one of our doctors will discuss your medical condition and history with you. If you're able to give the doctor access to your medical records, it is very helpful. This significantly increases the likelihood of getting your medical marijuana card. If you’re not sure how to obtain your records, we can assist you with that.
If your doctor is able to confirm your condition and believes medical marijuana would be an appropriate and effective treatment for you, you will receive a signed certification which recommends you use marijuana as a part of your treatment plan.
Required Forms and Information to Apply
After receiving a recommendation for medical marijuana from our doctor, you'll have to register with the state. The following are required when submitting your application to the state of Missouri:
• The completed application form
• The completed Physician Written Certification
• Photocopy of your Missouri-issued driver's license or state ID (please be aware that your name and address on your application form must match what is listed on your Missouri driver's license or ID)
• The nonrefundable $25 application fee to the state
Post-Appointment Information
The recommendation will need to be submitted to the Missouri Department of Health and Senior Services, which we can take care of for our patients. The State also requires a $25 annual fee for this submission, which you will be responsible for.
The State will then process this information and add you to the patient registry. Shortly after, you will receive your medical marijuana card!
